Free printable medical record log

Have you ever been filling out a form (as a new patient at a doctor’s office, applying for insurance, etc.) and were asked for your (or your child’s) medical history?  And do you usually have to go by memory because you never wrote it all down?  I do.

Well, I got tired of trying to remember it all (and I never could remember it all), so I made up this form to keep track.  I keep mine in my bag (along with the kids’ vaccination records).  Whenever I’m at the doctor’s office with someone (or myself), I fill it out while I’m waiting for the doctor to come in.  Then, any follow up info (like prescribed meds, date symptoms ended, etc.) can be filled out later (which I usually forget to do but I’ve found that as long as I have the date of the visit etc., I can usually get by or, I can always call the doctor’s office for the info if absolutely necessary).

    Ooh shoot i just wrote a big comment and as soon as i hit reply it came up blank! Please tell me it worked right? I dont want to submit it again if i do not have to! Either the blog glitced out or i am an idiot, the second option doesnt surprise me lol. many thanks for an excellent blog!
